What are your office hours?

Our Glendale and Cedarburg offices are open during the following hours:

Mondays-Fridays: 9:00 a.m.-5:00 p.m.

Are there evening hours?

Generally no, but please call us and check with our receptionist to learn the specific availability of each of our physicians.

Do you take my insurance?

While we do work with most insurance carriers in the metropolitan region, the specifics of many insurance policies and plans change frequently. To be certain of your insurance policy's limitations, we encourage you to check with your insurance carrier before arriving for your appointment.

Do I need to bring my insurance co-pay to my appointment, or will you bill me?

Insurance contracts require that co-payment be made at the time of services. Our office accepts cash payments, checks, MasterCard, VISA and Discover.

Why do I have to fill out all this paperwork?

Patients being seen in the clinic will be asked to fill out registration forms containing demographic information, as well as other paperwork regarding your medical history. The questions on these forms help us better serve you, as they provide information necessary to treat and manage your symptom.

Do I need to bring medical records from my previous physician?

In order for our physicians to provide a thorough evaluation, we request that all of your previous medical records (including examination notes dictated by your previous physician, operative notes and X-ray/other imaging reports), as well as X-ray films or MRI scans be obtained prior to your appointment. Please note that we can accept X-rays/MRIs on both film and disk. If you need assistance in obtaining your previous treatment records, please contact our office, and we will be happy to provide a release form for you to send to your previous doctor.

Is there a charge if I miss my appointment?

No, we do not charge for missed appointments; however, given the nature of our busy medical practice, we request that you notify us of your need to cancel or reschedule your appointment at least 24 hours in advance.

Is parking available at your offices?

Parking for patients is readily available at both of our locations.

Is there an elevator?

Our Glendale and Cedarburg offices are located on the main level, so an elevator is not necessary.
